About Wraparound
It is a community based facilitation
services for families to coordinate
care and improve overall
functioning in multiple life
domains.
Population Served:
Children and families who have
severe psychological, emotional
and/or behavioral struggles in
multiple life domains and
involvement with two or more
providers are eligible for
services.
Referral:
Children who currently have Medicaid,
DSM-IV diagnosis or possible
diagnosis, two or more providers
involved with family, struggles in
home, school or community.
Complete NIW referral form and
send to System Coordinator.
Services Provided:
Child and family centered teams
that are family driven, culturally
competent, provide family with
“voice and choice”, and includes natural
supports and coordinates care with
family. The Family Facilitator empowers the family to take
charge of their care and manage
all formal as well as natural
supports increasing a common
focus between family and
supports.
Network for Safe Families
A voluntary and free program to
families to gain home based case
management.
Population Served:
Families who are at risk of child
abuse or neglect are eligible for
service.

Services.
Services Provided:
Home based case management
provided by partner agency within
one of three service tracks:
Mental Health Additions; Social
Service; Mental Retardation
Developmental Delay. Family
assigned to track based on
presenting issue and home based
services provided by agency
specializing in that area of
service. For a list of providers
and referral sources, please
contact a partner agency in your
county or SCAN. Home based
services are approached with a do
for, do with, and a cheer on
attitude.
Community Alternatives for
Psychiatric Residential
Treatment Facilities
A combination of facilitation and
home based case management to
serve children in community
rather than in residential setting.
Population Served:
Children between ages of 6-20
who qualify for placement in a
long term Psychiatric Facility or
are who are currently placed in
long term Psychiatric Residential
Facility will be eligible for services.
Referral:
Child must qualify for Medicaid;
family must reside in county with
access point (NIW), between ages
6-20 and must have assessment
completed by access point (NIW).
Complete NIW referral for and
send to System Coordinator.
Services Provided:
Child and family will receive
Wraparound based Family Team
care and have access to such
things as flex funds, respite care,
training and support for unpaid
caregivers, transportation
services, habilitation services and
therapeutic services including
psychotherapy and case
management. Services can be
provided to family while child
resides in a psychiatric facility |
